If you can't retract it and able to clean it, you should see a urologist.
Sorry, but without more details, or a physical exam, I cannot say much about your particular issues. The most common disorder with penile foreskin is that it does not retract (that is pull back comfortably) and that is a diagnosis called Phimosis. I highly recommend that you seek consultation with a urologist.
