My penis itches most times it's painful and I have this on it.

HPV doesn’t itch or have pain.
Sorry but this is not a good quality picture for me to help you with a diagnosis. I highly recommend you seek consultation with a local dermatologist or urologist and in the meantime, either avoid sex or be careful - use a condom!
