I have hypothyroidism. Should I tell my ENT if I have chest pains?

Your ENT will likely not help. I would tell your PCP.

I would. He/She is still a doctor who can listen to your chest, take vital signs, feel your pulse. Unless you have documented cardiopulmonary disease, the likelihood of a serious problem is improbable. More likely musculoskeletal.
