I had a hemorrhoid come out a few days ago. It is very painful and is not responding to OTC medications, suppositories, and external cream. What should I do?

I recommend seeing your family doctor for an acute evaluation. You would likely benefit from prescription medications and possibly a referral to a colorectal specialist for surgical evaluation and treatment. If you have not been soaking your anorectal area in hot water several times a day ( a sitz bath) for 10-15 minutes at a time please do that as well.

Your treatment depends of the type of hemorrhoids. External hemorrhoids or internal hemorrhoids that has prolapsed out.
You need to see a Colorectal Surgeon. Meanwhile you can take Sitz bath ( sit in lukewarm water for about 15 minutes twice daily, make sure water is not hot if not it will burn you.) Take 25-35g fiber daily and drink 2-3 liters of water daily. stool softener and lidocaine gel OTC. If your pain is very severe, then go to the emergency room.
