My son is 6 years old and he has an earache. How do I know if my baby has an ear infection?

A doctor needs to look into the ear to determine if there is an infection, but generally, children also have nasal congestion and a runny nose when they have an ear infection (though this is not always the case).
He would have to have his ears checked by a doctor. Some infections are middle ear some or external ear and need different treatment.
