If both ankles are swollen it is usually a sign of a systemic problem which could be one of the following: heart, lungs, kidneys or central vascular problem. If one ankle is swollen it can be a sign of injury or vascular problem in that extremity,

This could mean venous insufficiency, heart failure, too much sitting in an obese person or unlikely or possible inflammatory arthritis.

If both ankles are swollen with no history of previous ankle injuries then most common reasons are inflammatory arthritis (rheumatoid, psoriatic, lupus), metabolic (mineral deficiency), cardiac, renal or vascular. Most common are traumatic or inflammatory especially if you have no other medical issues. Good luck.

Without knowing anything about your history: if it's both ankles and no trauma, may be an issue with your veins (part of the blood circulatory system that is responsible for bringing blood back to your heart), or your heart. Sometimes it is diet/nutrition related, too.

Since it is rare to sprain both ankles, especially given no history of trauma, it is important for you to see your primary care doctor for a work up since swelling in the ankles can be anything from poor circulation to other medical conditions.
