Plan your visits ahead of time
If your doctor has to spend all of the time in the appointment asking basic health questions, they will end up covering less ground than if you point them in a specific direction. In other words, they will not simply know your concerns and investigate accordingly if you are not able to communicate with them what is concerning you, or what has changed between visits.
As a general rule, tumors that are visible and growing are in later stages of the disease, depending on the size of the tumor and the person. You should be the one to ask for a mammogram, or at the very least, to ask when would be the best time to start getting regular screenings. If clinical breast exams are not offered at your regular location of care, you might consider asking your doctor where and how you can get screened regularly.
