Does being overweight make it harder for me to lose weight?
Being overweight for any extended period of time does mean that it will be harder for you than for naturally normal weight people to maintain a normal weight. Being overweight means that your “weight setpoint” which is the specific body weight that your body tries to maintain, will be permanently higher. What is the best weight loss pill for metabolic syndrome?
The best weight management medication for someone with Metabolic syndrome would be one that actually treats insulin resistance, like Saxenda. (Belviq also used to help with insulin resistance, but it was taken off the market.) That being said, any weight loss is better than none when it comes to Metabolic Syndrome, so if all you can afford is phentermine, that’s better than nothing (as long as your heart is healthy enough to use it.) Is maxillofacial surgery advisable for a diabetes patient?
It depends on how well her diabetes is controlled. If her A1c <7.0% she should be ok. If it is >8.5% she won’t heal well so the surgery should be delayed until her sugars are better controlled. READ MORE

What is the best way to stop bleeding in a diabetic patient?
Who told you diabetics are at a higher risk of bleeding? Patients with liver disease are at a higher risk of bleeding, and many Type 2 diabetics have fatty liver disease due to obesity, but for diabetics without liver disease, in general the opposite is true. They are at risk of blood clots and strokes because high sugars make the blood more viscous (thicker), and inflammatory cytokines make blood more likely to clot. This is one of the many reasons diabetics have a higher risk of heart attack and stroke. READ MORE
If I avoid medicines and concentrate on lifestyle, can I win over diabetes?
It depends on how much insulin your body is able to make. Have your doctor order a C-peptide level. If your c-peptide is high, then your diabetes is still in the reversible stage, and you can likely manage your diabetes with a combination of diet, exercise, and stress management. If your C-peptide is low, then your pancreas has been damaged to the point where you will not be able to go without medications entirely, but diet and exercise will still help minimize the number of medications you will have to take. Does insulin cause weight gain?
Yes. This is expected. Without Insulin, your body was unable to access the nutrition in the food you were eating so you were surviving by digesting your own fat and muscle.... and urinating out a great deal of sugar and calories. With insulin the body can absorb the sugar in food. It then burns this sugar to give you energy... what sugar is not burned will be stored away in fat cells for later use. What should be the ideal HbA1C for a diabetic patient?
This is a slightly more controversial question than you would expect. The simple answer is that you would like your A1c to be as low as possible without experiencing hypoglycemia. So if you could get to an A1c of 6.5% or lower without hypoglycemia, that would be great. However as people age, the risk of hypoglycemia becomes greater because it stresses the heart.... so for older individuals, endocrinologists generally use the rule of thumb of keeping A1c

When does medication start for diabetes?
140 is not borderline. It is full Diabetes. I start meds at a fasting sugar of 126 or higher. Sometimes even earlier. It takes at least 5 years of poor sugar control for fasting sugars to rise that much since the first sugars to rise are usually the after-meal sugars in the pre-diabetes stage.... fasting sugar elevations mean a great deal of pancreatic insulin production has already been lost. Do medicines lose their effectiveness if taken for a long period of time?
Yes. Drugs in the sulfonyuria class “glyburide, glipizide, glimepiride, glibenclimide” stop working after about 15-20 years because the pancreas’ ability to produce insulin gets exhausted after about 20 years of having diabetes. The pancreas scarrs down from prolonged inflammation and beta cells (which make insulin) die off over time. READ MORE
